I googled for a long time. But the closest I could find to a solution was this post:

...never mind. I found the answer through "try-on-error"

Figured it out... needed to select 'Start Menu' instead of 'Desktop' in the Shortcuts area in order to add a Start Menu shortcut.

[SOLVED] Create Start Menu Folder is greyed out in final executable.
